Team DC is soliciting nominations for the annual First Pitch and three new awards: The MVP Award, Trailblazer Award, and Athletic Support Award. The criteria for the First Pitch and each award is listed below.

• MVP Award: Presented for efforts throughout the past year to support a team, league, organization, and/or tournament.
• Trailblazer Award: Presented for work over a substantial period of time to help establish and grow a sport, club, and/or tournament.
• Athletic Support Award: Presented to a business, individual, or organization for consistent support of the local LGBT sports community.

First Pitch: Bestowed upon a leader for contributions to the LGBT community, this honoree will throw out the first pitch at the 4th Annual Team DC Night OUT at the Nationals. Nominees should have a long-term, demonstrated depth of commitment and contributions to the LGBT community.

Nominations shall be limited to one page and shall include the following information:

• Name of nominee and contact information (address, phone number[s], and email)
• Affiliation of nominee (team membership, if any, and community involvement)
• Name of nominator and contact information (address, phone number[s], and email)
• Summary of accomplishments related to the award nomination (no more than 300 words)

Nominations for the awards and the First Pitch are due no later than 5 P.M. on JUNE 1, 2008, and must be submitted by e-mail to Vince Micone, Chairperson—Team DC Nominations Committee—at dcvince3@gmail.com. Awards will be presented at a ceremony in July. The First Pitch will be thrown at the 4th Annual Night OUT at the Nationals on June 23, 2008.
